You may activate your membership using the online registration form and complete your payment using Visa or MasterCard.

Did you know…

1. With the exception of Student Memberships, when becoming member you may select the auto renewal option. Simply ensure your credit card and membership category is current and your membership will be renewed automatically on May 1st.
2. If you are joining on or after March 1st your membership will be valid through April 30th of the following year.

Select your membership category:

• Full Member – $210.00
• Retired Member – $80.00
• Resident Member – $70.00
• Student Member – $25.00
• *Associate Member – $130.00

* FMWC welcomes Associates who may be male or female and need not be a member of the medical profession. Associates do not have voting privileges.
